Long Beach Veterans Service Center

Congress has passed a new law that offers COVID-19 vaccines to more Veterans and some spouses and caregivers of Veterans.

Enrolled Veterans

Veterans currently enrolled in VA health care can schedule their vaccine appointment by calling 562-826-5300.

Non-Enrolled Veterans and Veteran Spouses

Veterans or Veteran spouses not currently enrolled in VA programs will need to fill out the 2021 VHA SAVE LIVES Act COVID-19 Vaccination Worksheet and either:

  • Bring it to our Enrollment and Eligibility staff located in vaccination center
  • Fax it to: 562-826-8133
  • Mail it: Tibor Rubin VA Medical Center, Eligibility Office, 5901 E. 7th. St, Long Beach, CA 90822

Once submitted, we will schedule their vaccine with any available same day appointment or if unavailable, the next appointment available (usually within days). To expedite enrollment, we ask all non-enrolled Veterans to bring their DD-214 or any VA Benefit letter. If they do not have these documents, every attempt will be made to verify Veteran status through VA Records Systems.

Veteran Caregivers

Veteran caregivers can call 562-826-5300 to either schedule their vaccine appointment or be helped with the process.

Eligible Veteran Beneficiaries

Survivors and dependents of certain Veterans under the CHAMPVA Program as defined in 38 U.S.C. 1781 are also eligible for vaccination. They should call 562-826-5300 to either schedule their vaccine appointment or be helped with the process.
